Attending Physician
The attending physician plays a vital role in every patient’s care. Working closely with the hospice team, they oversee all aspects of medical management to ensure each patient’s comfort, safety, and well-being. In most cases, the patient’s regular doctor continues to guide their plan of care, maintaining continuity and trust throughout their time in hospice.
Hospice Chaplain
Our chaplains provide spiritual care for patients and their families. Whether you seek prayer, reflection, or simply someone to listen, they’re here to support your spiritual and emotional well-being. They can also connect you with your own faith leaders to ensure your personal beliefs and traditions are respected.
Patient Care Manager
The patient care manager is a registered nurse who oversees and coordinates all aspects of each patient’s hospice care. They ensure every plan of care is individualized, comprehensive, and responsive to changing needs. Serving as a central point of contact, they communicate closely with the physician and care team, helping families feel supported and informed at every step.
Certified Home Health Aids and Nursing Assistants
Certified Home Health Aides and Nursing Assistants provide gentle, hands-on support with personal care such as bathing, grooming, and other daily activities across the Greater Los Angeles area. Their visits also give family members time to rest, knowing their loved one is in capable, compassionate hands. Visit frequency is tailored to the needs of both the patient and the caregiver.
Bereavement Coordinator
The bereavement coordinator provides guidance and emotional support for families coping with loss. They work closely with loved ones to create a personalized plan for grief care, offering counseling, resources, and a steady presence throughout the healing process. With understanding and empathy, they help families find comfort and meaning in the days ahead.
Volunteers
Hospice volunteers offer companionship, comfort, and emotional support to patients and their families. They bring warmth and presence through simple acts of kindness, listening, sharing time, or providing a quiet moment of care. Volunteers also give caregivers a chance to step away, rest, and recharge while knowing their loved one is supported.
Hospice Medical Director
The hospice medical director collaborates with the attending physician and care team to ensure each patient receives the highest standard of attentive medical oversight. They provide clinical expertise, thoughtful guidance, and a broad perspective that supports the coordination of individualized care across the entire hospice program.
Social Worker
The social worker offers emotional support, counseling, and practical guidance for patients, families, and caregivers throughout Southern California. They help navigate important decisions and connect loved ones with community resources, financial assistance, and supportive services. With empathy and understanding, they ensure no family faces the challenges of hospice care alone.
Foot Care
Foot care is an important part of the comfort and dignity we provide to every patient. Our team offers gentle nail trimming, skin assessment, moisturizing, and monitoring of the feet and lower extremities. This service is especially beneficial for patients with diabetes, poor circulation, or limited mobility, helping prevent discomfort and skin breakdown while promoting overall well-being. Foot care is included as part of the hospice Medicare benefit at no additional cost to patients or families.
