What happens in the final days of hospice care?

As a patient approaches the end of life, our hospice team increases the frequency of visits to provide closer monitoring and more intensive comfort care. Nurses watch for signs of active dying, including changes in breathing, circulation, and consciousness, and communicate openly with families about what to expect.

Medication adjustments are common during this time. Our team focuses on eliminating any discomfort, managing labored breathing or agitation, and ensuring the patient is as peaceful as possible. Families are guided on how to administer comfort medications if needed between nurse visits.

We encourage families to be present, speak to their loved one, and create the environment that feels right to them. Hearing is believed to be one of the last senses to fade, and many families find meaning in reading aloud, playing meaningful music, or simply holding a hand. Our team supports whatever approach brings peace to your family.

After a patient passes, our nurse will visit the home to pronounce the death and support the family through the immediate hours that follow. We will contact the appropriate authorities and funeral home, and provide guidance on next steps. Bereavement support continues for 13 months after the loss.
