When to Consider a Hospice Referral

A hospice referral may be appropriate when a patient meets one or more of the following criteria:

Terminal prognosis of six months or less if the disease follows its expected course

Frequent hospitalizations or ED visits in the past 6 months

Progressive functional decline despite optimal treatment

Patient or family expresses desire to focus on comfort over curative treatment

Significant uncontrolled symptoms: pain, dyspnea, nausea, or anxiety

Progressive unintentional weight loss, recurrent infections, or PPS ≤50%
